Subject: Important Update: Office Visit Pricing Changes Effective January 1st

Dear Patient,

We hope this message finds you well. As always, we deeply value the trust you place in us to support your health and wellness journey. Since we opened our doors six years ago, we have been committed to providing you with the highest quality care with few adjustments to our pricing. However, due to rising operational costs, we find it necessary to make a change to our office visit rates.

Effective January 1, 2025, the cost of an established patient office visit will increase to $300. This adjustment ensures that we can continue offering personalized and comprehensive care, which includes meeting one-on-one with our Health Coach and Nutritionist, as well as with Dr. Williams during your visit. Visits with other mid-level providers will be charged at $250 per visit. Please note: Clients who are in a membership or an active wellness program (Bronze, Silver, Gold or Platinum program) will not be responsible for any additional fees for an office visit as these are included in your plan.

To remain an active patient, it is necessary to schedule an office visit and lab work at least once every six months. Patients who have not been seen within 11 months will need to re-establish their care with us by scheduling a New Client Consultation.

To provide additional value and convenience, we encourage you to explore our membership options, starting as low as $99/month. These plans are designed to support your ongoing health needs while offering exclusive benefits:

• Two annual office visits included

• Unlimited access to patient portal messaging and health coaching telehealth visits

• 10% discount on supplements

• 10% discount on laboratory fees not covered by insurance

• No lab draw facility fee

• Some membership levels also include Bioidentical Hormone Replacement Pellet Therapy (BHRT) options
